LICENSE APPLICATION PROCEDURE

 

Business License

  • Fill up the business license form provided
  • Complete the form
  • Attached the required documents
  • Pay the RM 20.00 process payment

Advertisement License

  • Complete the advertisement / sign board / banner / poster application form
  • Attach a copy of your identity card
  • Make payment

Night / Flea Market License

  • Complete the application form
  • Attach a copy of your identity card
  • Pay the RM 20.00 process payment

Canine License

  • Complete the license application form
  • Attach a copy of a veterinary examination from a qualified veterinarian
  • Attach with a copy of your identity card
  • Photo of your pet dog.

Seasonal License

  • Send an official letter to MDSR
  • Identify the business premise
  • Attach with a copy of your identity card
